Barrier Enclosure during Endotracheal Intubation

This video from Boston Medical Center that shows what happens to medical staff when intubating a suspected COVID-19 patient with and without the use of an aerosol intubation box.

** The video uses a mannequin that simulates a cough and uses blue light to show the spray onto medical staff with and without the box.

OUR DESIGN – Model “a”

Living Light Shows was called upon by the medical community to lend our expertise in computer-aided plastics fabrication and develop an improved style of intubation box that addressed the deficiencies of existing solutions in the market.

Together with the help of Anesthesiologists and Respiratory Therapists who are actively treating patients with COVID-19 we developed a design that provides significant improvements, functionality, and protection to reduce the risk to healthcare workers by containing the aerosols generated by the patient.

Model “A” has rigid sides and thereby does not require any plastic sheeting or covering to provide protection to the rear or the sides.

MODEL "A" - Overview of Features

Model "A" - Side/Front View
Model "A" - Doctors View
  • Seamless thermoformed transition between the back and the top of the box to increase physicians visibility to the patient.
  • Side handles outside the contaminated area of the box allow easy movement by healthcare workers without reaching fingers around inside the box.
  • Pear shaped access holes with outward sweep and wider diameter at the lower position maximize natural movement of arms around a head and when reaching forward under the chin.
  • Side cut out for assistant to have reach access while still benefiting from shielded protection.
  • Slanted top towards physician improves provides better visibility.
  • Only three surfaces with minimal inside corners to ensure easier cleaning and maintenance.
  • All corners are curved and edges softened to prevent inadvertent scratching or tearing of PPE.

Model "B" - Overview of Features

Model "B" - Front/Side View
Model "B" - Doctor View
  • Completely open sides and front offers maximum movement around and inside to patient.
  • Raised bottoms on sides and back to facilitate setting over hoses, tubes, lines, etc.
  • Lip all the way around the top to use squeeze clamps (or binder clips) to hold disposable plastic drape.
  • Seamless thermoformed transition between the back and the top of the box to increase physicians visibility to the patient.
  • Pear shaped access holes with outward sweep and wider diameter at the lower position maximize natural movement of arms around a head and when reaching forward under the chin.
  • Slanted top towards physician improves provides better visibility.
  • Only one large surfaces with minimal inside corners to ensure easier cleaning and maintenance.
  • All corners are curved and edges softened to prevent inadvertent scratching or tearing of PPE.

“Considering the spread of contaminants a single cough, every measure should be taken to contain droplets and protect health care providers… One should consider adapting this concept, even if adequate PPE is available.” Department of Anesthesiology, Boston Medical Center, Mach 28, 2020

OUR DESIGN – Model “b”

This is our second model of Intubation Box. This one uses disposable plastic over the rigid Plexiglas frame, It offers much of the same benefits as Model A except for the sides. It has better accessibility so the RT can reach with both hands beneath the draped plastic on the sides and front as needed to maneuver an ambu-bag, hold the jaw up, lift face and make proper seal of mask on patient face for pre-oxygenation, etc. There is no rigid plexiglass hindering their movement around the front and sides which is why it requires the consumable plastic cover.

This shield allows health care workers to perform swab testing and other diagnostics on potentially contagious patients with an additional layer of security. The large thermoformed plexiglas wall with pass-through sleeves allows contact-less interaction with less PPE and reduced risk by inhibiting exposure from droplets expelled by the patient from coughing and sneezing.

The sleeve ports have a concentric ring design allowing disposable sleeves to be held on with a large rubber band and quickly changed. There are a variety of options for the sleeves including using only the disposable sleeves, using latex/nitrile gloves over the sleeves, or using surgical gown sleeves.

Donations / Sponsorships

Time is of the essence and we are looking for donations to help ramp up our production capabilities as well as subsidize the costs of thermoforming and building these units and getting them into the hands of hospitals and medical facilities as quickly as possible, at as low of a cost as possible.

We have started a GoFundMe campaign to receive these donations. The funds will be used on modified tooling, jig creation, materials, labor and distribution/shipping costs. Please consider partnering with us on this project.

Who Are We?

Lights Alive - Christmas Lights San Antonio Texas

Prior to this pandemic our company Living Light Shows, focused solely on designing and fabricating displays for musically synchronized Christmas light shows for commercial venues. Our latest project, Lights Alive! is a nearly mile-long Christmas light drive-thru venue in San Antonio, TX, that required many of the same robotic manufacturing machines and plastic forming expertise that is needed to produce these intubation boxes. Unhappy sitting on the sidelines during this pandemic as a “non-essential” business, we want to put these machines, our staff, and our expertise to use for our healthcare community.
